Preliminary Damage Assessment Form
Use this form to report structural damage on your property in the wake of the August 10, 2020 storm.
This form is intended to collect information on structural damage to homes, businesses, garages, sheds, etc. This form does NOT take the place of an insurance claim. The report will help the City's Building Services department quickly identify areas that sustained damage and assess community needs.
NOTE: To improve accuracy, please select "Straight-Line Winds" under the Primary Cause of Damage field. Please also ensure the "Date of Event" field is set correctly to 8/10/20.
