How to get a copy of a crime report:
To obtain a copy of a police record you must complete a ‘REQUEST FOR RECORD' form. This form can be obtained at the Cedar Rapids Police Department or downloaded RequestForRecord.pdf. The completed form, along with payment (see fee schedule below), may be dropped off or mailed to the Cedar Rapids Police Department, 505 1st St S.W. , Cedar Rapids , IA 52404 .
Requests for records may be made in person at the Cedar Rapids Police Department 8AM-3PM, Monday-Friday.
Completed requests returned by mail require a self-addressed stamped envelope.
