- The City will submit this completed form to the Iowa Division of Criminal Investigation.
- Required for both new and renewal applications.
- A separate form is required for each name being checked.
- If you have used the same last name for the past five years, the DCI will check only the last name you are currently using. Thus, a maiden/married name situation (less than five years) would require more than $12.00 for the City to make an email request for a records check.
- For any corporation that has members not residing in the state of Iowa, the members will supply their own criminal history information to the Cedar Rapids City Clerk’s Office. This needs to be from a source authorized to check criminal histories.
- The checks are needed for any officers, directors, and shareholders who own 10% or more of the corporation (an investigation will have to be done on the manager if different than the owner).
