The City will submit this completed form to the Iowa Division of Criminal Investigation.
One form is required for each technician/installer
- i.e. anyone who will be entering homes or businesses in Cedar Rapids on behalf of your company for installation and/or maintenance of equipment
Alternately, you may provide a copy of the individual's criminal history from your local authority, obtained within the last year. |
