New License Application for Alarm Maintenance / Monitoring Business	$100	Same form is used for new license and for license renewals.
Renewal License Application for Alarm Maintenance / Monitoring Business	$50	Same form is used for new license and for license renewals.
Alarm Identification Card Application	$21.50	One form is required for each technician/installer. i.e. anyone who will be entering homes or businesses in Cedar Rapids on behalf of your company for installation and/or maintenance of equipment An identification card from the Alarm agent's company will be accepted as long as the identification card contains the following information: Recent photo of applicant Card issue date The applicant's signature The card is valid for 1 year from the date of issue All technicians and installers must have their pictures taken and identification cards typed up and laminated at the City Clerk's Office. Criminal History Records Check Request Form	$12.00	The City will submit this completed form to the Iowa Division of Criminal Investigation. One form is required for each technician/installer i.e. anyone who will be entering homes or businesses in Cedar Rapids on behalf of your company for installation and/or maintenance of equipment Alternately, you may provide a copy of the individual's criminal history from your local authority, obtained within the last year.
Certificate of Liability Insurance meeting the following requirements		In order for an insurance policy to be approved by the City Attorney, it must meet the requirements of the City Council. These requirements are as follows: An original Certificate of Insurance or Surety Bond with an original penned ink signature of the agent writing the policy (certificate will be returned if it does not have an original signature) The name, address, and telephone number of the agent signing the policy should be typed under his signature. "The City of Cedar Rapids, Iowa, and its employees are named as additional insured" needs to be specifically indicated on the policy. A copy of Agent's Insurance License or other document showing the Agent's authority to sign for the insurance company must be attached to the certificate. The liability limit required by the City is $500,000. The name of the specific job being done for the City or license applied for, needs to be stated on the Certificate or Surety Bond. Address for the Producer and Insured cannot be a PO Box. An identification card from the Alarm agent's company will be accepted as long as the identification card contains the following information: Recent photo of applicant Card issue date The applicant's signature Please forward this information to your insurance company. Any exceptions to these requirements must be specifically granted by the City Council and made known to the City Attorney.
